Voice Your Opinion About SAMHSA's Disaster Preparedness and Response Services
Disasters and emergencies can happen anywhere at any time. SAMHSA's Disaster Technical Assistance Center (DTAC) supports SAMHSA's efforts to prepare states,
territories, tribes, and local entities to deliver an effective mental health and substance abuse (behavioral health) response to disasters.
You have been selected to participate in an important web-based survey on your experiences with SAMHSA DTAC because you have either requested technical assistance from SAMHSA DTAC in the past or you are subscribed to the DTAC Bulletin or The Dialogue. This survey takes about 15 minutes to complete.
We want to hear your thoughts about SAMHSA DTAC services and products so that we can continue to provide you with high-quality disaster preparedness and response materials and services. Please complete the survey by April 28, 2015.
If you have any questions about the survey or technical difficulties, please contact us at 1-800-305-3515 ordtac@samhsa.hhs.gov.
Please complete this important survey today!
