Resources Help Keep Students with Diabetes SafeAs students return to school, it’s important to remember that students with diabetes face unique challenges related to managing their diabetes. Visit NDEP’s Diabetes Resources for Schools and Youth for resources to help children and teens manage their diabetes and stay safe at school. Also check out NDEP’s Transitions from Pediatric to Adult Caretool to help students with diabetes, their parents, and health care professionals transition from pediatric to adult health care.
Exploring Diabetes HealthSenseThe NDEP’s Diabetes HealthSense online tool provides resources to help people make changes to live well – whether or not they have diabetes. Find resources such as CDC’s Bam! Body and Mind, which gives kids ages 9 to 13 the information they need to make healthy lifestyle choices.
PPOD (Pharmacy, Podiatry, Optometry, and Dentistry) New Tool Kit and WebinarsHelp your patients with diabetes get the care they need. Too many people with diabetes are not getting their annual foot, eye, and oral exams, or managing their medication. You have the power to change that. The enhanced Working Together to Manage Diabetes Toolkit empowers PPOD professionals and other providers to deliver more integrated care for patients with, or at risk for, diabetes.
Learn from PPOD professionals who have worked collaboratively in their communities to better care for patients at risk for diabetes and those who already have diabetes using PPOD tools. Register for one of three webinars by following the links below.
